I am working on a website project to help psychologists and therapists record clinical notes, assessments and treatment plans. Part of the initial assessment involves the diagnosis of the patient including the problems and definitions. We would like to pre-populate problems and definitions with standard values. What books or papers would be good to use for a source for the standard values?
