We used to use Survey Monkey a lot for this kind of thing, but we finally got fed up trying to match subjects and score scales. Finally, we ended up writing our own software. It is designed for psychology researchers instead of marketers.

You can create users and match their surveys over time. And instead of scoring things after the fact, you can just download the scored results.
